No, it is just the choke as pictured.

would this be good for lead rifled slugs and 00buck and #4 bukcshot aswell?

I am fairly certain that slugs should only be shot from open cylinder, aka no choke. The buck shot would be fine, it says it is good for steel shot and heavy shot.

Well that's choke fit my Mossberg Maverick 12 gauge

I would think it should. Mossberg Maverick 88 barrels are interchangeable with the 500 series. I used this choke in a 535 12 gauge and it’s perfect.
