9006 would be the correct FOG LIGHT bulb for your car, not headlamp.

One of the bulbs was broken from the housing when I opened the box. I had the garage install the good one. Is there anyway of having one sent to me?

You need to contact Amazon customer service. Having the good one installed was not a good choice for multiple reasons... besides having mismatched lights, once you opened the package it’s pretty tough to prove that it arrived broken, and that you didn’t damage it trying to install it.

What’s the bulb life?

I've had them in my truck for about 3 years now, if that helps.
