Will this fit a large size man for back ??

If he has a large waist, you will have to get an extender that has velcro attachments. It's small for my 40" waist, just barely fitting.

How long does it stay cold?

I didn't time it. However I needed to ice after surgery for several days and I thought it stayed cold a pretty long time.

Great, stays cold. Soft velvet side. Multiple uses

Brilliant. Great for wrapping round back, stomach, knee, arm. Anything that anyone in the house bumps or hurts this comes out. It has a nice velvet soft side so fine again direct skin. Every home should have in the freezer much better than a bag of peas. Stays cold for a long time too. Elastic Velcro strap is nice length too would fit around people of all sizes, children and very large men.

Comfortable, well designed ice pack. Does the job.

This is a well designed ice pack for the back and shoulders. Soft, velvety on one side which you can reverse for a colder treatment on the other side. The velcro fastened harness is long enough to tighten to whatever length you need and the ice pack doesn't slip when I attach it to my shoulder. Good product for the price.

Decent large ice pack

Having had 2 ACL repairs + 1 arthroscopy, I know a thing or two about sports ice packs.Balanced review to help buyers.Pros - it’s large, soft and comfortable, easy to apply. Can be worn with fabric side touching skin (less cold) or turned reverse so the non-fabric side touches (colder).Cons - takes a long time to freeze and once out, the coolness seems to disappear relatively quickly (eg 20-30mins).Overall a decent buy. However for the price cannot give 5 stars. Hope this helps.
