Q: Does this have metal in it?

Hi there. The only metal is on the rivets that hold the velcro on the side.

Q: Would a size small accommodate a calf of 16 inches?

Our short air walker boot is just based on shoe size. There should be enough space to accommodate your calf size.

Q: I have an injured metatarsal. Does this boot shift the weight onto the heal or onto the toes? Could you walk putting your weight on the heel only?

I used mine while I had a broken ankle, so I cannot specifically answer about a foot injury. I think it would be possible to keep the pressure on the heel only, but that might make your knees & calves tired or uncomfortable after a while. The boot does not allow your foot to bend or move at all while walking or standing.
