How do you apply this? Would I need to buy a sprayer separately?

Yes, you need a sprayer. This is concentrated so you need to mix with water. I mix in a 1 gallon sprayer. Follow the mixing and spraying instructions on the bottle.

Do you have to spray the entire shrub or just around the base?

I sprayed the entire shrub and whatever fell on the ground and it did kill the bag worms I was amazed but yes I sprayed the whole bush very heavily

Is this a concentration? How much of this to add to a gallon of water?

It is concentrated. 2 ounces per gallon. Or a little more depending on what you are spraying for. Mine was a 32 ounce bottle
