Q: Are these Velcro adjustable

Yes the Velcro gives plenty of adjustment to fit the width of your feet.

Q: size and fit - I am a size 12 but I know some US shoes come up larger. Would a size 11 suffice?

I am a uk9 or European 43 USA 10. They are true to size in my opinion. I have had lots of Reef flip flops over the years. I love them the sliders are probably my all time favourites. I hope this helps
