How does one get the castors to screw into the holes of the bottom shelf?

You have to push it in. It doesn’t screw. Might take a little muscle.

Rotatable castors missing. How do I get them from Amazon without returning the whole thing?

I would contact them and see if they can just send you the casters. They are good about replacing items that aren't right.
