What do you measure to get the right size, your foot in inches or your calf in inches?

Your calf. The foot is generous so if you have a size 12 wide foot these should fit. At least the 3x and 4x did for me as I wear a woman 13 EEE and a man 12-13 wide depending on the cut.

Anyone tried these under ski boots? How is the thickness?

I personally have not used under ski boots but have used them for running. So, it seems to me that, they would make a good underlayer for any oversock.

Can you sleep in these socks

I do not sleep in mine. I wear them 2 or 3 days and take a day off. They work best for me using like this.

I'm planning to use these for after my c-section when my legs and feet will be swollen, should I size up?

YES! Size up. I originally got a 2x didn't fit well. Then did a 3x and the fit perfectly!
