can these be sewn on versus hook and loop? Thanks.

They can be but it is not easy to do since they are a thick plastic. I ended using mounting tape and a few stitches to make sure they stay on.

These are great little velcro patches, but were they supposed to come with separate hook back to stick on a surface?

Mine didn't. Most med pouches already have soft loop somewhere for mounting.
