Q: Is it okay for a 5'11" person to do sit-ups?

Yes, my son is 6 feet 3 inches tall and it works great.

Q: Can the height be adjusted?

The height of the bench itself can't be adjusted, but the back can. The back can be adjusted to make the bench flat. A pin can be pulled to fold it up to a more compact position as well.

Q: Just got mine and set it up. The 2 bolts that go in the foot supports don't tighten. Blue washer in nuts prevents that. Is that intentional?

That's a nylon lock tight washer, you should be able to tighten the nut till you can't,
