What are the actual dimensions? Because the 3.5 " thing amazon states seems too big. I'm guessing that might be the packaging.

Is there a fan inside this power supply unit?

Yes it does. It has an intake fan that faces front when it is installed. The PS in the Optiplex 390 died. This unit went in and the machine has worked perfectly every since.
