Yes it does come blank. It has the stickers and ribbons on a separate sheet as well (except there is one stuck to the very top sheet in the box which is kind of annoying).

Do the gold seals come seperately? Or are they already on the certificates?

The gold seals are on a page that is shrink wrapped inside with the certificate papers.
