How does this compare to neilmed which has salt and bicarbonate, but does not have xylitol?  and what is xylitol?

Hello!  Great question.  As you mentioned, XLEAR contains xylitol which is the primary difference.  Xylitol is a natural, 5 carbon sugar alcohol that's derived from the fibrous parts of fruits and vegetables and has several interesting properties.  There are two benefits the xylitol provides in the upper respiratory system.  First, it makes the XLEAR solution hypertonic.  This means the solution draws excess moisture out of swollen tissues, which reduces inflammation and opens your nasal passages allowing you to breath easier.  Second, xylitol is non-fermentable by many of the harmful bacteria strains responsible for upper respiratory infections.  This creates an environment for the bacteria that is difficult to proliferate and thrive.  So, like NeilMed, XLEAR contains saline and sodium bicarbonate for a general cleansing of the nasal passages and sinuses; however, the addition of xylitol increases the effectiveness at a deeper level.

This is advertised as 100 packets. The photo shows a box of 50 packets and the “details”   say it is a spray. What do I get if I order this, exactly?

They make sprays ,also 20 count, 50 count, and 100 count individual packet boxes. You must read the items that are all separate in order numbers. The individual packets can be used in a netti pot. The sprays are ready to use. I keep on hand some of each for year round use.

What is the free sinus spray sample that is included?

The sample is a small .25 fl oz. squeeze bottle of the XLEAR Nasal Spray with Xylitol.  While the Sinus Rinse Bottle provides exceptional relief from congestion and mucus build-up, XLEAR only recommends periodic use of the positive pressure rinse due to its ability to completely flush away the built-up mucus.  This helps alleviate congestion but leaves the delicate sinus tissues exposed to bacteria, pollutants, irritants, etc..  The nasal spray sample is included as it helps rebuild a healthy mucus layer and can be used more regularly to thin mucus and reduce inflammation.
