Can this be used on a bed?

Yes, it can be used on the bed but it's very hard and uncomfortable my back was sore after using it!

Will I be able to place this in a way that will work on a shorter trunk? Also trying to target lower lumbar and sacral spine.

For stretching the back: Place the pillow on a hard surface, preferably a carpet Then lie on the pillow and feel your back stretch It shouldn't be painful - if you feel pain, move the pillow to a position that will be comfortable for you on one side, but you will feel a stretch on your back on the other. In order to reduce back pain, lying on the pillow twice a day for about 5 minutes is recommended. You can find many more uses for the Yesindeed Back Stretcher Pillow, including: - Foot Massage - Legs Lifting - Neck Lift & -Stretch - Chair Support Enjoy your new product Best wishes from Yesindeed
