Will this fit a mobility scooter

Really don't know. Too many variations. In theory yes if the spindle size is correct and tyre size is correct. Items are great. Check your user manual if you have one or go on line to get one. It should also say on the scooter tyre. Can really say without seeing it.

Do the wheels come with the caps that keep them from falling off?

No they do not come with end caps. Normally there is a washer and a split pin that is already on the trolly axel that is used to prevent the wheel coming off.

What is the definitive size of bore? One answer says 20mm the other says 25mm
