Does the stated length (186cm) include the pillow or is it just the length of the mattress?

It is the total length, including the pillow. The mattress length is about 150 cm

does the pillow self inflate too?

The pillow is inclusive of the sleep mat but it's inflated via a valve on the side. Even though the mat is self inflatable you still have to top it up via your lungs👍 🤣

Is there an R rating for this mat

It doesn't state what the r rating is but I've used mine in temperature down to around freezing and its very insulated.
