National Semiconductor LMC6484AIN OP Amp Quad General Purpose Amplifier, Rail to Rail I/O, 15.5V, 14-Pin, MDIP Rail, 6.35 mm W x 3.3 mm H x 19.05 mm L (Pack of 2)

One of the two ICs was defective. Upon close examination there were burn marks on pins 1,2 and 3 indicative of being in contact with a very high voltage source. The rest of the pins were clean and functional. The op amp pins 1,2 and 3 did not operate.Thankfully the second of the two ICs was fine I needed just one IC immediately.
