Do the brushes rotate with the rifling groves?

Old question, but answering as the other responder is wrong. Do not pull these through the muzzle. Pull from chamber. Yes, they will rotate. The Otis system is the best I’ve found.

What's the point of the 12" and 20" cable for pistols?

These are for specialty pistols with longer barrels, for example colt buntlines and modern handheld stockless pistols. Imaging something like the FN-15 ("stockless AR-15"), but in 9mm to .45 caliber.

So I have a brush for the .22, but will the cable itself fit down the bore of a .22LR?

Yes - the cables with the female ends will fit. Note, however, that .22 caliber bores are notoriously tight, so the .17 caliber brush is recommended.
