Most all gems have a size discrimination of mm millimeters & a photo of gems around a quarter. The sizes you've listed are teeny tiny, look farther in product description.

Can be used on teeth?

These object is small. We don't recommend to use on teeth.

Are these hotfix rhinestones? Do they already have a glue on the back od the rhinestone?

You DO have to glue these. These are NOT hotfix.

what is the pen for???

It's not a pen, it's kinda like a crayon. You are supposed to use it to pick up the gems. You up glue dots or whatever onto item you're putting gems on, then use crayon/sticky pencil on face of gem to pick up & place flat back on adhesive/glue. Let dry or cure (try not to mess with for 24 hours, or as directed) then rub off any residue left from crayon.
