Q: Will this fall through my lip if I use it as a lip ring?

It is very tiny. It's a possibility. I use it in my ear but and it hasn't been even close to happening. However, the lips are used a lot more and are in more contact with things that might pull it.

Q: Do you think that this can be used as a nose jewelry or is it too thick?

Generally, a 20-22 gauge works best for the nose. There some really nice nose bones here that are in those sizes. I use a 22 gauge myself. It keeps the piercing hole less noticeable if I want to take it out for a bit.

Q: Does the gem top stick up and out from the surface? The prongs make me thing it would be laying flat/flush with surface

No , not at all. I ve bought it last summer n still using it. Had no issue.never bothered me.
