It's made from flannel under the poly fleece, it is washable, but I wouldn't dry it just air dry to keep it soft

Will this fit OVER the face cradle pillow of my Fitmaster? It's mine exclusively so won't need a cover.

I'm not familiar with the Fitmaster table's face cushion, sorry. However, this product will fit most standard sized face cushions. I measured a face cushion in our office and it's circular and about 11" long by about 3.5" wide, so give or take a inch each way and it should fit.

is this item machine washable?

Yes, it is machine washable. I recommend only washing with whites, as dark lint tends to cling to the fabric.
