Q: Can you remove the hook to stand the light on a table?

You can’t remove the hook, but you can fold it, and it does stand on a table.

Q: does this include batteries?

Yes! :) They worked great on my camping trip. Had them on for hours each night and never had to change the batteries.

Q: How do you switch between high and low modes?

There’s a button on the colored part, you click it a few times to change the color and intensity!

Q: Where can I get a replacement for the battery holder?

Not sure that you can. These light are cheap enough, just replace the entire light. You realize that the battery holder is reusable, right?
