Does this have screw terminal connections?

Yes, take a small flat screwdriver to pry the plug apart and inside you will find the screw connections. This is my favorite replacement plug. I've used it many times.

Specs mention it has an led, but pictures don't show it. is it lit when powered?

I don't see an LED unless it is in the dark horizontal slot shown in the picture of the Electrical Plug. Mine has that dark horizontal slot as shown, on two sides, but mine doesn't light when plugged into an electrified receptacle (powered), and doesn't light when the lamp itself is turned on. I'm not sure what the point of having an LED would be in this case, or if in fact it even has an LED. This makes me doubt the dark horizontal slot I see really is an LED (light emitting diode). Also, I note the description of the device "Leviton 48643 000-000 2 Prong Non-Polarized Electrical Plug, 125V, 15 A, 2 P, 2 W, 1 Pack, Brown", doesn't mention an LED.

I need a plug for a ceiling fan with 3 lights. Will this work?

Ceiling fans typically use a 3 wire connection.

Has anyone used this plug on 16/2 hpn wire for a space heater and did it hold up?

I used this product on a table lamp using a 100W incandescent light bulb, and plugged into a standard household 110V, 15 Amp circuit. Later I changed to an LED bulb that draws significantly less current. My answer is "No", I have not used it in the circuit you describe to run a space heater that I would think would draw significantly more current. No, I have not used the plug in the application you describe. When used properly, the product does seem to be well made and yet has the appearance of a plug in use in the 1920's, which was important to me for use with my father's table lamp high school shop product from that era.
