How many epoulets does it come with?

It comes with two, and they are very well done!

How are these attached?

The epaulets are like sleeves, there is a wide gap in between the two sides, they perfectly slide over the shoulder strip. You unbutton the shoulder strip, slide the epaulets and button it up again.
