what are the size of the connection openings ?

Gates 10MB-8MJ - G60301-1008 will fit, This is if your wanting -8 an size. If you want a -6an size get Gates 10MB-6MJ - G60301-1006 .

What size are the port openings? Are they female 1/2" -14 NPT? And they are in the center of mount correct? What size are the 4 bolt holes?

I know the mounting bolt holes use a standard 5/16" bolt from home depot, but the female inlet/outlet threads are not NPT. I used an ORB (o-ringed boss) fitting that I picked up from my local speed shop. No leaks so far but IIRC they make the same filter head but it will accept a 1/2" NPT thread, I found that info on one of the diesel forums (I think it was a dodge forum). If you can find that part number, I would go with that one to simplify getting the fittings you need. But yes, the ports are centered on either side
