will this work with a 18" 3 speed floor fan. that runs no more than 4 hrs or less at a time?

Irregardless of the type of fan, as long as it runs on standard 110Volt it should work. I am using it on me ceiling’s fan which plugs right into a 110V socket. I got it so I can control my 3speed fan more precisely. Works well for me.

Can this fan controller be used on a gfci protected circuit?

Should be able to use on any 110 outlet. Used mine daily this last summer with a large fan and worked perfectly

There is no cord accompanying the device. It has the standard 110v male and female plugs. I plugged the male into the wall plug and my fan's cord into the female plug in the device. It is an excellent product and I can recommend totally.

There are no markings nor label to indicate country of origin but given the very low price, I would say it's a safe bet that it was made in China. Wherever it was manufactured, it works well and controlled my exhaust fan very well.
