The dimensions are a bit confusing. My toilet seat is 4" thick ... will this fit on there or will it be too big?

It fits a standard toilet sit, but you may lift the sit, and try it that way since the measurements are different.

I love the concept, but my brother is 51 years old and heavy set. it is hard to get it in between his legs. he is down syndrome. needs to be a thinner

It is a little wide, we use tape on the sides to keep it from rubbing his inner thighs. Just masking tape. Works very well at keeping the urine in the toilet.

will this splash guard stay on a square toilet seat?

I don’t think it would. But can’t be sure. It didn’t fit on my square seat but mine was too thick so it wouldn’t stay on
