Does this fit on to a breaker bar?

No, it doesn’t fit directly on a breaker bar. I took a pic of my Kobalt set that mine fits onto, I just don’t know how to include it.

Will this fit 2016 dodge dart

We had a 2013 Dart and it had 19mm lug bolts. I never experienced any issues with them swelling under the stainless caps so I didn't need this socket for that car as a standard 19mm socket was all I needed. If your Dart does have corroded and swollen lugs this should work.

Is this made to handle "swollen" lug nuts? The standard 19mmlug nut wrench provided by Ford for our 2014 Escape will not fit over the existing lug nut

Yes it will help getting off the swollen lug nuts but you should replace them because they will keep swelling
