2 players, play through the game cooperatively on the same system.

The game includes the multiplayer update on disc or does it have to be downloaded?

There's a small update when you install the game. I'm unsure as to what all it includes. I believe the multiplayer is already on the disc, however.

The local coop mode (available a few days before physical release) will be included on disc or added through download?

I believe the physical edition was printed / mastered before the 2 player patch went live, so you'll need to have internet and a PSN account to update. The back of the PS4 game box says 1 player only if that's any indication.
