How does it get moist (I want moist heat for my dry eyes)?

I add a few drops of water before heating it up. I have another moist one and it does not behave any differently. This item is a quality item

I use my mask twice daily, but you should check with your doctor to see what he recommends.

what is the fabric and what does it feel like?

It is a polished cotton. I like the feel. I have not needed to wash it yet but the beads do come out for washing. I have to microwave it for a minute with a new wall microwave even though the directions say 30 seconds. After 30 seconds it is not even warm. So far it has not caused a problem. Inside there appears to be ceramic beads instead of seeds like some others types
