Is the shaft meant to have a slight bend in towards the middle of the shaft, 15 inches up the staff? Box came smashed.

No. The putter shaft is bent to the designed lie angle about 3 inches above the putter head. If you look down the shaft from the top of the grip, like looking down a rifle barrel, the center of the putter shaft bisects the putter face. Sounds like they need to replace the putter.
