CMS 1500 Claim Forms "New" HCFA (Version 02/12) - Health Insurance, Laser Cut Sheet - 500 Sheets

Same forms used at medical practices normally purchased at big box stores for close to double the price! Excellent quality, no issues with printing. Can't beat the deal on Amazon unless you find a local independent supplier who will price match. Fortunately, I did, but this will be my back-up.

Perfect these forms worked seamlessly with our billing program and were great quality! I had purchased these forms a few months ago from a DIFFERENT seller and the paper was thinner, red ink streaked on the back,and our template was always slightly off when importing to the forms. This seller was the best! The forms were on thicker paper, high quality! Would HIGHLY recommend!

It is a great value, will definitely purchase again

We have ordered these in the past from this seller and been fine, but this latest batch the QR codes were all blurred and the when we printed the claims, the printed text did not line up correctly in the boxes. I checked with another form from an old batch that printed fine, so it is not a printer problem. It seems like a bad batch. Unfortunately I didn't open the package until after the return window.

The claim forms are really light in print compared to previous orders. Almost pink instead of red.

As described, prompt shipping, good quality, works perfect for getting claims paid.
