Bought these repair order forms for use at my home shop. They are an excellent value at about $9. for fifty repair orders. Space at top left for company stamp. Has space for parts - labor - access. - haz. waste, etc. They are made in the USA :) And supposed to be for use in all 50 states so they may have some lines/ statements you won't use. But should work well for for a small shop / business. Follow up 9/21/2023 : I have decided to stop using these repair orders because the bottom third of the already small form is wasted with unneeded statements. I am going to buy the 100pk of 8.5" x 11" 2-part auto repair order form by Beck's Printing, cost more .37 ct. vs .18 ct. per form, but larger and without space wasted on statements you don't need.
