We're in the process of auditing our Amazon product listings and came across your question. Thank you so much for posting it. I apologize though that no one responded to you sooner. All TENS devices need two connection points to create a circuit. Without this circuit the current can't be distributed and your device won't work. So in the case of your cuff, yes, you will need to pair it with another electrode on the second wire of your Healthmate Forever. Most commonly people use the cuff in conjunction with a conductive electrode sock or glove. A second cuff will also work, as will most larger adhesive electrode pads. We recommend pads that are at least 3" in diameter in order to correctly balance the current between the two connection points. Otherwise you were correct to attach the cuff directly to the lead wire of your Healthmate device. The included adapters are for use in connecting the cuff to lead wires that use metal pin connections instead of the snaps. I hope this helps. **Q: I want to use it around thigh, which is 22 inch. Will it fit?** A: No **Q: How do you use it for earthing? Does it connect to the wall plug?** A: Yes, you connect the cuff to the round ground receptacle at the bottom of a wall outlet (not to the slatted electrical holes). To do this you will need an Earthing cable which IS NOT INCLUDED in this package. NO ELECTRICITY is exchanged in grounding or Earthing so you will not feel the buzzing of current in the electrode garments as you will with an electrotherapy device. The garments do however allow you to experience the many benefits of Earthing or Grounding. You can find a number of Earthing cables here on Amazon by entering "Earthing Cable" or the following product ID into the Amazon Search Bar: B00ZVFQR7O. To attach the gloves to the Earthing cables you will unsnap the short white snap adapter that comes pre-attached to our electrode garments. Your Zynex NexWave unit should be compatible for use with Conductive Therapy Shop brand electrode garments, including our conductive cuff packages. The NexWave should ship with lead wires that have US standard-type split lead wires with black and red metal pin connectors that attach to female receptacles on electrodes of various descriptions, including our CTS electrode cuffs.It is important to note that when using a single electrode garment with a system with split lead wires such as the NexWave sold by Zynex, one of the pins will be attached to the electrode cuff and the second pin on the lead wire will need to be attached to either a second electrode garment, or to a sufficiently balanced adhesive pad that will be affixed to the lower arm or leg. This allows a circuit to be created. In the single cuff package you are inquiring about we include a 3" round adhesive pad designed to properly balance the current with the large surface area of the cuff. Beyond the ability to connect to our products, different TENS devices can produce different intensities and different electrotherapy sensations during treatment that are generally device specific and may be influenced by battery life, lead wire condition, treatment set up, application of electrolyte spray or gel, age of the device, etc. Conductive Therapy Shop electrode garments themselves will function regardless, but they are simply accessory items to electrotherapy devices such as the NexWave and thus their performance can be greatly influenced by these other factors. They definitely DO help. I have ended up using the cuffs more because they are a thicker material and hold the moisture longer which is needed for conductivity. I originally intended to use the cuff on my calf with the sock but the cuff is much too small for that to work for me. So, I wear the cuff on my foot, just below the toes, and attach the pad that came with the socks to my leg. I use this as my occupational therapist instructed, alternating with the use of the conductive electrode glove by the same company.
