Q: how is the blade attached to the handle? is it bolted on the inside?

The handle is hollow so the blade has a short tang that is pressure-inserted into the handle. Most people , me included, who buy a hollow-handle knife with a short tang put a strong glue at the bottom of the handle to keep the blade in place. A gel like glue (like Gorilla) would work.

Yes - most of the little items come packed in a container in the handle. It's pretty sweet!

Q: What's the handle made out of? Is it durable?

I don't know 100%. The handle is fairly weighted, so it might be made of an aluminum alloy, but I think there is some steel in it too. It is strong enough to handle being run over by a vehicle like those flashlights in the commercials. Though honestly that is a terrible way to demonstrate the strength of a cylinder as many plastics can withstand that type of test. Just don't use it as a hammer and you'll be fine.
