Q: Does it spill when flipped over? Thanks

No spills when the bottle is upside down or flipped around. My little one has thrown this around and banged it multiple times and it still works as it should- no spills (you do have to make sure that the seal is in place properly after washing and filling).

Q: Do they have to bite the spout for the water to come out?

No, they just need to suck. The spout is connected to a straw that goes down to the bottom of the beaker.

Q: Can the straw be remove to clean?

Yes it can. You lift the top part of the straw up and out of the clips and then pull it through the bottom. The booklet that comes inside it shows you how
