Q: What pen works for this? Mine are too wide? Also, why is it missing r and R?

Thin points like the micron type pens are best. A capital R can be made from the capital P, but not sure why the r is missing

Q: Is this made for use with a rapidograph pen?

Yes, Rapidograph pens work with this style of lettering guide, but so do modern drafting markers, available in any office supply or art supply store. These latter don't have the sometimes messy ink filling problems, but, if you're careful, you mat find these pens with ink that's both waterproof and permanent, just like India ink.
