Q: Will this fit over a heavy duty extension cord?

Yes. I have a thick yellow cord from a heavy duty outlet strip going though it.

Q: How do you hold the cover down to hard floor?

Many thanks for your interest in D-Line Floor Cord Cover, they are made from a flexible material which simply uncoils to lay flat (with no need for adhesive tapes).
