Q: is this hat fully lined with fleece?

No. Just the inside headband. This is my favorite winter hat and I will buy it again if this one goes bad.

Q: What country is it made in ?

The one I have was made in China.

Q: Will it fit someone with a large head?

Somewhere, the description states this hat is, "one-size fits all." Much too small for me, I gave it to someone whose noggin is much smaller than mine. It didn't fit them. I went back and reread the description and found a sizing chart… It showed the one size fits all hat with the same dimensions as a Small/Medium size. The chart also indicated a large size, but there were no large hats. Ended up returning the hat. It may have fit my cat, wish I had thought to check.
